Why this compound has a literature
Lysine's research literature is shaped by one fact: it is the first limiting amino acid in wheat, rice and most other cereal staples. That gave it a public-health nutrition literature — fortification field trials in cereal-dependent populations — that is unusual for a supplement-grade amino acid, and a well-developed requirement-estimation and bioavailability literature using stable isotope methods. Alongside these sits a much older and much weaker clinical literature on herpes simplex prophylaxis, and a substantial animal-nutrition literature that accounts for most global lysine tonnage.
What the literature investigates
Cereal fortification field trials
Because lysine is the first limiting amino acid for protein utilisation from wheat, several controlled fortification trials were run in populations obtaining most of their protein from cereal. A three-month double-blind study near Peshawar, Pakistan enrolled 80 families, in which wheat provided 58-65% of protein, and reported greater increases in children's weight and height in the lysine-fortified group, along with changes in haemoglobin, transferrin and prealbumin. A parallel three-month randomised double-blind study in poor communities in northwest Syria reported reduced plasma cortisol response to blood drawing in females, reduced sympathetic arousal by skin conductance in males, and reduced trait anxiety scores in males. These are population-specific studies conducted in settings of marginal lysine intake; the findings describe correction of an inadequate baseline intake and cannot be generalised to populations with adequate protein intake. A 2025 review of lysine for childhood and adolescent growth surveyed this literature and set out precautions.
Requirement estimation and food-matrix bioavailability
Lysine is one of the most thoroughly measured amino acids in the indicator amino acid oxidation (IAAO) literature. A 2024 study in 30 healthy adults over 60 reported requirement estimates stratified by age and sex, giving a mean of 32.9 mg/kg/day (upper 95% CI 40.9) for women aged 60-69 and 46.2 mg/kg/day (upper 95% CI 53.7) for women over 70, against a current recommendation of 30 mg/kg/day derived from young-adult data. Separate IAAO studies have measured lysine bioavailability from specific food matrices, reporting 94% bioavailability from moist-cooked sorghum in five healthy young men. This literature is methodologically strong but small in sample size, and it addresses dietary adequacy rather than supplemental intake. It is nonetheless the most defensible technical basis for discussing lysine dosing in fortification applications.
The herpes simplex prophylaxis literature and its limits
Lysine's best-known consumer association rests on a body of trials mostly published between 1978 and 1984, with conflicting results — some reporting subjective improvement, others reporting outright failure. A 2017 evidence review of oral L-lysine for herpes simplex recurrence concluded that supplementation appears ineffective for prophylaxis or treatment at doses below 1 g/day without a low-arginine diet, that doses above 3 g/day appeared to improve patients' subjective experience, and that longer controlled studies at doses exceeding 1.2 g/day are required to test the question definitively; the review also flagged theoretical cautions for patients with cardiovascular or gallbladder disease. The veterinary analogue was tested more rigorously and came out negative: a 2015 systematic review concluded that lysine supplementation was not effective for prevention or treatment of feline herpesvirus 1 infection in cats. Formulators should treat this area as unresolved and note that claim wording here is closely scrutinised in most markets.
Tolerance and the dose ceiling in the intervention literature
Lysine has an unusually well-organised safety literature for an amino acid. A 2020 systematic review pooled 71 intervention studies covering 3,357 subjects, doses up to 17.5 g/day and dosing periods up to 1,095 days, and reported that observed adverse events were mainly subjective gastrointestinal symptoms, with a risk ratio of 1.02 that was not statistically significant, and identified a provisional no-observed-adverse-effect level of 6.0 g/day. A general review of amino acid supplement side effects separately discussed competition among amino acids for shared membrane transporters and degradative enzymes, which is the mechanism most often raised when a single amino acid is dosed well above dietary levels. Together these define the range within which the published human literature actually operated.
What this means when you specify the material
Two numbers anchor most lysine specification discussions. The intervention safety review pooled doses up to 17.5 g/day and identified a provisional NOAEL of 6.0 g/day, and the IAAO requirement work sits around 30-46 mg/kg/day depending on age and sex. Note that this SKU is the hydrochloride at roughly 82% lysine base, so any dose taken from the literature — which is almost always expressed as free lysine — must be converted before it is applied to a label or a fortification calculation. The fortification trials used lysine added to flour at dietary-adequacy levels, not supplement-scale doses.
