体重与身体成分的荟萃分析
这是临床证据中体量最大的一块。Talenezhad 等(2020)合并 37 项随机对照试验、2,292 名受试者,报告体重加权均数差为 -1.21 kg、BMI 为 -0.24 kg/m²、脂肪量为 -2.08 kg;腰围与体脂百分比无显著效应。仅纳入高质量试验时只有体重结果得到确认;其非线性剂量-反应分析提示 2,000 mg/日为模型效应最大点。作者本人将该效应描述为「幅度不大」。评估这支文献时应注意:试验人群高度异质——超重与肥胖成人、透析患者、多囊卵巢综合征女性——且亚组结果并未在所有人群中一致成立。
运动表现与运动强度这个变量
Mielgo-Ayuso 等(2021)在 Nutrients 发表的系统综述刻意按运动强度分组而非直接合并,考察了中高强度下急性与长期文献各自使用了哪些剂量与给药时机。Sawicka、Renzi 与 Olek(2020)采用了更严格的纳入标准——健康受试者、口服肉碱至少 12 周、不合用药物或复方产品——最终只有 11 项研究合格,剂量为 1-4 g/日、持续 12 或 24 周。该综述报告肉碱与碳水化合物合用可提高骨骼肌总肉碱含量,而 24 周补充未影响健康老年女性的肌肉力量。符合基本方法学标准的研究数量如此之少,本身就是这一领域值得注意的特征。
长期补充与 TMAO 这条研究线索
Koeth 等(2013)在 Nature Medicine 报告:肠道菌群将膳食 L-肉碱代谢为三甲胺、继而生成氧化三甲胺(TMAO);杂食人群在肉碱负荷后产生的 TMAO 多于纯素与素食人群;在小鼠中,长期膳食补充肉碱改变了盲肠菌群组成并加速动脉粥样硬化,而在肠道菌群被抑制时该效应不出现。Sawicka 等(2020)随后指出,长期补充肉碱可升高人体空腹血浆 TMAO,但其考察的炎症与氧化应激标志物并未随之改变,并呼吁开展更长期的心血管研究。这仍是一个悬而未决的领域而非已有定论的结论;设计长周期产品的配方人员或宜持续关注。

盐酸盐规格约提供 68% 的 L-肉碱碱基,而已发表的剂量几乎都以肉碱碱基而非盐来表述,因此标签声称与临床对照必须采用同一折算基准。口服试验的剂量集中在每日 1-4 g 肉碱;负载类研究采用了与碳水化合物同服的方式,这属于配方层面而非原料层面的变量。由于盐酸盐呈酸性且吸湿,水分指标与包装是实际采购中的关键控制点;饮料类应用需评估其对体系 pH 的影响。
